Heat olive oil in a skillet over medium-low heat. Add onions, season with kosher salt and black pepper, and cook, stirring occasionally, for 20 to 25 minutes until the onions are golden and soft. Set aside.

In a large bowl, combine lean ground beef, ketchup, Worcestershire sauce, onion powder, kosher salt, and black pepper. Mix gently until just incorporated. Divide and form the mixture into 4 patties slightly larger than the bread slices.

Melt 2 tablespoons of unsalted butter in a cast-iron skillet over medium heat. Add the patties and cook for 4 to 5 minutes on each side until browned and cooked through. Transfer the patties to a plate and set aside.

Wipe the skillet clean and melt the remaining unsalted butter over medium heat. Place 4 slices of rye bread on a clean surface. Top each with one cooked patty, a portion of caramelized onions, and 2 slices of Swiss cheese. Cover each with a second slice of bread.

Place assembled sandwiches in the skillet. Grill for 2 to 4 minutes per side, pressing gently with a spatula, until the bread is crisp and the cheese has melted.
